At Brookside Inn, our Admissions Team works closely with hospitals, physicians, patients, and families to ensure a smooth transition from the hospital to our facility. From the moment we receive your hospital referral, we're here to guide you every step of the way.
As you consider your post-hospital care options, we encourage you to learn about the services, support, and environment available at Brookside Inn. For patients and families who have the opportunity to visit before admission, touring our community can provide a firsthand look at daily life, therapy services, and resident care. During your visit, take time to observe interactions between staff and residents, the atmosphere of the community, and how residents engage in daily activities. Seeing the community in action can help you feel more informed and confident in your decision.
Before you Arrive our Admissions Team Will:
- Coordinate with your hospital's discharge planner or case manager
- Review your medical needs and physician orders
- Verify insurance coverage and benefits
- Answer questions about your stay and services
- Prepare for your arrival to ensure your room and care team are ready
- Assist with admission paperwork
What to Bring:
- Photo ID
- Insurance cards
- Comfortable clothing and shoes
- Eyeglasses, hearing aids, dentures, or other assistive devices
- Advance directives or healthcare power of attorney documents
Insurance & Payment Information:
We accept a variety of insurance plans, including Medicare, Medicaid, Humana, United Healthcare, and many other commercial insurance providers. Coverage authorizations and benefits vary by plan, and our admissions team is available to help verify benefits and answer questions about coverage and potential out-of-pocket costs.

Frequently Asked Questions Prior to Admission:
1. Who arranges transportation from the hospital?
Transportation is typically coordinated by the hospital's discharge planner or case manager. Our Admissions Team works closely with the hospital to help ensure a smooth transition to Brookside Inn.
2. Do I need to bring my medications?
In most cases, you do not need to bring your medications. The hospital will provide an updated medication list at discharge, and medications will be supplied by the facility based on your physician's orders and treatment plan. If there are any medications you should bring with you, our Admissions Team will discuss this with you before your admission.
3. How much therapy will I receive?
Therapy services are based on your physician's orders, clinical needs, individual recovery goals, and insurance coverage. Your therapy team will evaluate your needs and develop a personalized treatment plan following admission.
4. How do you meet my nutritional needs?
Our Dietary Team prepares well-balanced meals designed to meet your nutritional needs and physician-ordered diet. We accommodate a variety of therapeutic diets, food allergies, and texture-modified diets, and our registered dietitian works closely with your care team to help ensure you receive safe, appropriate nutrition. In addition to three daily meals, snacks and beverages are available 24 hours a day. Whether you need a snack between meals or a drink overnight, our staff are always available to assist.
5. What happens before I am discharged home?
Before discharge, our interdisciplinary team works with you and your family to develop a safe discharge plan. This may include arranging home health services, ordering recommended medical equipment, coordinating follow-up appointments, and providing education to help you transition home successfully if your needs have changed and returning home is not appropriate, we can assist with finding an alternative care setting.
