Brookside Inn, a family-owned skilled nursing and rehabilitation facility in Castle Rock, is seeking a compassionate and conscientious 30-hour-a-week discharge planner / admissions support to join our team of qualified individuals. We are looking for a professional who is committed to the highest standards of integrity, quality care, and respect for our residents.

In this role, you will work directly alongside the Director of Admissions, providing vital support to ensure our facility maintains seamless 7-day-a-week admissions coverage. You will act as the primary point of contact for admissions while the Director is out, ensuring no referral or new resident transition is delayed. Simultaneously, you will serve as a navigator and advocate for our short-term rehab residents—organizing care conferences, managing insurance timelines, and ensuring a safe, comprehensive transition home.

Note: This is an hourly position requiring a flexible schedule, including availability for at least one weekend day per month to maintain our commitment to weekend admissions and discharge excellence.

Rate of Pay

  • $27.00 to $30.00 per hour based on experience

Responsibilities

Discharge Planner (Rehab Hallway)

  • Interdisciplinary Care Coordination: Organize and lead weekly care conferences with the IDT, skilled patients, and their families to review progress, ensure goals are being met, and proactively resolve barriers to discharge.
  • Clinical Partnership: Work alongside the Rehab Case Manager (RN) to synchronize clinical milestones with discharge timelines.
  • Insurance Advocacy & Compliance: Manage the NOMNC (Notice of Medicare Non-Coverage) process and facilitate insurance appeals by submitting clinical documentation to carriers.
  • Transition Planning: Coordinate all post-discharge needs, including Home Health, Durable Medical Equipment (DME), and follow-up medical appointments.
  • Psychosocial Assessments: Perform assessments for all residents and maintain frequent contact with families to assist with the adjustment to the facility.

Admissions Support

  • Admissions Coverage: Partner with the Director of Admissions to ensure 7-day-a-week coverage; assume responsibility for the admissions process during the Director’s absence.
  • Rapid Referral Response: Maintain quick responsiveness to all incoming referrals, processing documentation as it arrives to ensure a competitive turnaround for hospital partners.
  • Facility Onboarding: Complete all necessary admissions paperwork with residents and families, ensuring accuracy and regulatory compliance.
  • Community & Provider Relations: Maintain strong working relationships with hospital case managers and outside referral partners to ensure a steady census.
  • Marketing & Outreach: Participate in marketing events and meet with community members to promote facility services.
  • Other duties as assigned

Qualifications

  • Education: Bachelor’s Degree in a Human Services related field (Social Work, Psychology, etc.) (preferred).
  • Experience: 2+ years in Long-Term Care, Discharge Planning, or Admissions (preferred).
  • Responsible, patient, and compassionate, Excellent customer service skills
  • Strong understanding of Medicare, Medicaid, and Managed Care/HMO insurance processes, including NOMNCs and appeals.
  • Superior interpersonal and conflict-resolution skills; ability to deliver complex insurance information with compassion and clarity.
  • Ability to make independent decisions and solve complex discharge hurdles in a fast-paced environment.
  • Ability to assess residents and relate information regarding resident’s condition.
  • Proficient computer and typing skills; ability to navigate Electronic Health Records (EHR).
  • Ability to communicate and function productively on an interdisciplinary team.
  • Must be able to read, write, speak, and understand the English language
  • Demonstrate dependable, regular attendance
  • Must be able to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours.
  • Ability to communicate policies, procedures, regulations, reports, etc., to personnel, residents, family members, visitors, and government agencies/personnel
