Rate of Pay

  • $46,000-$54,000 annually, depending on experience

Qualifications

  • Previous experience in a skilled nursing facility and a passion for memory care!
  • Previous management experience
  • Must be able to speak, read, write, and understand English
  • Current activity professional certficate or current unencumbered Colorado Occupational Therapy license, or a Therapeutic Recreation Certification having at least one year of experience in providing activity programming in a nursing care facility.
  • Must possess the ability to deal tactfully with residents, family members, visitors, staff, and the general public.
  • Must be willing to undergo a criminal background check

  • Must be a supportive team member, contribute to and be an example of teamwork

Responsibilities

  • Interview and assess each resident’s activity needs
  • Plan, schedule, and implement a program of individual and group activities based on residents’ schedule
  • Determine the resources needed to each resident’s activity needs and obtain approval to obtain necessary items from the Administrator
  • Assess and complete the sections of the Minimum Date Set assigned completely, timely, and accurately
  • Gather data and document required ancillary assessments including but not limited to resident assessment protocol summary documentation
  • Identify and document in the resident’s care plan, the activity interests, needs, and concerns of the resident
  • Document all interation with resident and/or family in the assessment, care plan and progress notes as required by federal and state requirements
  • Plan and conduct Residents’ Council meetings
  • Plan and implement evening and weekend functions as necessary
  • Recruit, schedule and supervise assistants and volunteers
  • Organize and schedule community events related to residents’ interests
  • Maintain activity attendance record for each resident
  • Supervise activities assistants, complete staff schedules to ensure proper coverage, complete annual evaluations, and other supervisory responsiblities
  • Be responsible for the safety of residents under Director’s supervision
  • Perform other dutes as assigned
