Rate of Pay
- $46,000-$54,000 annually, depending on experience

Qualifications
- Previous experience in a skilled nursing facility and a passion for memory care!
- Previous management experience
- Must be able to speak, read, write, and understand English
- Current activity professional certficate or current unencumbered Colorado Occupational Therapy license, or a Therapeutic Recreation Certification having at least one year of experience in providing activity programming in a nursing care facility.
- Must possess the ability to deal tactfully with residents, family members, visitors, staff, and the general public.
- Must be willing to undergo a criminal background check

Responsibilities
- Interview and assess each resident’s activity needs
- Plan, schedule, and implement a program of individual and group activities based on residents’ schedule
- Determine the resources needed to each resident’s activity needs and obtain approval to obtain necessary items from the Administrator
- Assess and complete the sections of the Minimum Date Set assigned completely, timely, and accurately
- Gather data and document required ancillary assessments including but not limited to resident assessment protocol summary documentation
- Identify and document in the resident’s care plan, the activity interests, needs, and concerns of the resident
- Document all interation with resident and/or family in the assessment, care plan and progress notes as required by federal and state requirements
- Plan and conduct Residents’ Council meetings
- Plan and implement evening and weekend functions as necessary
- Recruit, schedule and supervise assistants and volunteers
- Organize and schedule community events related to residents’ interests
- Maintain activity attendance record for each resident
- Supervise activities assistants, complete staff schedules to ensure proper coverage, complete annual evaluations, and other supervisory responsiblities
- Be responsible for the safety of residents under Director’s supervision
- Perform other dutes as assigned
