Brookside Inn, a privately owned, skilled nursing and rehabilitation facility in Castle Rock, is seeking a compassionate and conscientious full-time Medical Appointment Scheduler and Transportation Driver to join our team of qualified individuals. Rate of Pay

  • $23.00 -$25.00 per hour based on experience
  • CNA license eligible for $26.00 per hour

Qualifications

  • Unencumbered Colorado Certified Nursing Assistant license preferred
  • Colorado Driver’s License with demonstrated safe driving history with a clean driving record
  • CDL license is greatly encouraged.
  • Ability to apply for CDL in the future required.
  • Previous experience working in a skilled nursing facility & caring for patients
  • Previous experience driving a facility vehicle and managing an appointment scheduling
  • Must be able to speak, read, write, and understand English
  • Must possess the ability to deal tactfully with residents, family members, visitors, staff, and the general public
  • Must be willing to undergo a criminal background check & drug screen
  • Must be organized and detail oriented

  • Basic computer skills

Responsibilities

Appointment Scheduling:

  • Serve as the primary point of contact for all resident medical and personal appointments.
  • Schedule, confirm, and manage appointments with healthcare providers, specialists, and other necessary services.
  • Communicate with residents, their families, and staff to coordinate and finalize appointment details, including date, time, and location.
  • Maintain an organized and up-to-date master schedule of all resident appointments.
  • Proactively monitor and track appointments to ensure they are not missed or double-booked.

Transportation Services:

  • Safely and punctually transport residents to and from scheduled medical, dental, and personal appointments using the facility vehicle. Assist with transporting new admissions and current residents back and forth from different hospitals or other local agencies.
  • Assist residents with boarding and exiting the vehicle, ensuring their comfort and safety.
  • Follow all traffic laws and safety protocols, including securing residents’ mobility aids (e.g., wheelchairs, walkers) during transport.
  • Maintain the cleanliness and good working condition of the facility vehicle, reporting any maintenance needs or issues to management.
  • Complete and maintain transportation logs, including mileage, resident names, and appointment details.
  • Operate the facility’s transport van in a safe manner; ensuring the bus is clean with a full tank of gas each day. Alerting the facility administrator when the vehicle needs to be serviced, inspected and registration renewed.
  • Ensure any new orders from outside physicians are given to charge nurse to enter resident chart.

Communication and Documentation:

  • Communicate appointment details and any necessary pre-appointment instructions (e.g., fasting) to residents and relevant staff members.
  • Document appointment outcomes, including notes from healthcare providers or any changes to a resident’s care plan, and relay this information to the appropriate nursing staff.
  • Maintain strict confidentiality of all resident information in compliance with HIPAA regulations.
  • Report any changes in a resident’s condition observed during transport to the nursing staff immediately.

Other:

  • Maintain confidentiality with resident information in accordance with HIPPA compliance and laws.
  • Implement transportation systems to meet patients’ needs in compliance with federal, state and local requirements
  • During down time, assist with stocking resident rooms with necessary supplies in an orderly and neat fashion
  • Assemble items when necessary
  • Consistently lifting/moving objects up to 170 pounds
  • Cleans equipment in compliance with infection control procedures
  • Distributes and stocks supplies to appropriate locations
  • Assist with administrative duties such as answering phones, greeting staff/visitors, etc
  • Performs other duties as assigned
